Consumer Rights & Records Access
As required under Texas House Bill 4224 (effective September 1, 2025), this page provides instructions for consumers to request their health care records, contact the licensing authority for our clinicians, and file a consumer complaint.

Requesting Your Health Care Records
You have the right to request a copy of your health care records. To request your records from Align Counseling PLLC, you may:
- Submit a written request by mail or email using the contact information below
- Call our office directly
- Send a message through your client portal, if you are an active client

Please include your full name, date of birth, contact information, and a description of the records requested. Additional identity verification may be required before records are released. Requests are processed in accordance with applicable Texas law and federal privacy regulations (HIPAA).
